To be considered for admission at Waukegan Healthcare Training Center, all applicants must meet the following requirements:
Applicants must be at least 18 years of age at the time of enrollment.
Education Requirement
A High School Diploma or GED is required.
Applicants must provide a copy of the diploma or GED certificate along with official or unofficial transcripts.
Identification Requirements
Applicants must submit one or more valid forms of identification, including:
State Driver’s License or State Identification Card, and/or
U.S. Passport, or Valid Visa, or Foreign Passport (accepted).
A valid CPR certification is required for enrollment and participation in clinical training.
If the applicant does not currently hold a valid CPR certification, CPR training is available through Waukegan Healthcare Training Center.
To ensure student and patient safety, applicants must provide:
Vaccination records, including:
Hepatitis B vaccination (or signed declination, if applicable)
PPD (Tuberculosis) test results (current)
All documentation must be current, legible, and verifiable.
Failure to submit required documents may delay or prevent enrollment.
